Change to Health Care Facility License Issuance

Print Friendly, PDF & Email

AFL 21-10 From the California Department of Public Health

February 22, 2021

TO: All Facilities

SUBJECT: Change to Health Care Facility License Issuance

All Facilities Letter (AFL) Summary

This AFL notifies all facilities that effective March 1, 2021, the California Department of Public Health (CDPH), Centralized Applications Branch (CAB), will change the way it issues licenses to health care facilities.

To increase efficiency and reduce the carbon footprint, CAB is implementing a paperless process for issuing health care facility licenses.

Beginning March 1, 2021, CAB will be replacing CDPH’s current practice of mailing a paper copy of the license to the licensee and will begin issuing licenses to health care facilities as a Portable Document Format (PDF). The license will be sent to the facility via email using the licensee email address reported to CDPH. If there is no licensee email address on file, the license will be emailed to the facility’s general contact email address reported to CDPH.

Facilities must continue to post the license, or a true copy thereof, in a conspicuous and prominent location accessible to public view. If you have questions about this AFL, please contact CAB by phone at (916) 552-8632 or by email at CAB@cdph.ca.gov.

Sincerely,
